Description
CHG gel pad conforms around the needle. Stays in place and in contact with skin. Transparent dressing and CHG gel pad permit clear visibility around the insertion site area. Flexes with patient movement and can be worn up to 7 days.

- Pharmacy or wholesale distributor license issued by the state board
- License address must exactly match the shipping address

For Individual Practitioners:
- Doctor, dentist, or nurse practitioner license issued by the state board
