Description
High performance poly bags are yellow for instant identification as chemotherapy waste containers. Additionally, both hamper and collection bags are labeled Chemotherapy Waste. Optional cardboard inserts with bag attached give additional stability and increased ease of handling. Hamper bags measure 25 x 34.

- Pharmacy or wholesale distributor license issued by the state board
- License address must exactly match the shipping address

For Individual Practitioners:
- Doctor, dentist, or nurse practitioner license issued by the state board
- Address does not need to match, but license must still be from the same state as the shipping destination
