Description
Specially designed for the left, right or bilateral amputee. Clack leatherette zippered cover. Leg extension are approximately 1/2 of the seat width x 9"L. Overall length with leg extension is 25", Overall thickness is 1" for the Narrow Adult, Adult and X-Wide models and have a 5/8" plywood base. The XX-Wide have an overall thickness of 2" and have a " plywood base. Meets CA-117 code.

For Business Accounts:
- Pharmacy or wholesale distributor license issued by the state board
- License address must exactly match the shipping address
- License must be from the same state as the shipping address
For Individual Practitioners:
- Doctor, dentist, or nurse practitioner license issued by the state board
- Address does not need to match, but license must still be from the same state as the shipping destination
Note: A DEA license will not meet the requirement.
