Packs include: Tiburon extremity drape with 28" x 45" IsoBac absorbent reinforcement; 2 1/2" circular elastic fenestration; cord-holding tabs. Mayo stand cover, reinforced polyethylene, 23" wide. Suture bag. Plastic sheet, fan-folded, 44" x 60". Outer wrap/reinforced poly table cover, 50" x 90", with 30" x 90" reinforcement.

- Pharmacy or wholesale distributor license issued by the state board
- License address must exactly match the shipping address
- License must be from the same state as the shipping address
For Individual Practitioners:
- Doctor, dentist, or nurse practitioner license issued by the state board
- Address does not need to match, but license must still be from the same state as the shipping destination
