Items included in both pouch and boxed versions: pair sterile exam gloves, disposable plastic apron, 17" x 24" underpad, 23" x 36" receiving blanket, 48" x 36" drape sheet, absorbent towels, OB towelettes, sterile 4" x 3" gauze sponges, sterile #10 scalpel, sterile OB pad, sterile 2 oz bulb syringe, alcohol prep pads, sterile Posi-Grip umbilical cord clamps, clear polybag with tie, white polybag with tie.
Accepted Licenses
For Business Accounts:
- Pharmacy or wholesale distributor license issued by the state board
- License address must exactly match the shipping address
- License must be from the same state as the shipping address
For Individual Practitioners:
- Doctor, dentist, or nurse practitioner license issued by the state board
- Address does not need to match, but license must still be from the same state as the shipping destination
Note: A DEA license will not meet the requirement.
