Sterile tray contain all necessary items for incision and drainage procedures. Each tray contains: 1 Polylined Fenestrated Drape, 18 x 16, 1 Polylined Towel, 1 Plastic Bag w/tie, 2 Povidone-Iodine Prep Pads, 4 Cotton-Filled Sponges, 1 Scalpel, #11 Blade, 1 Straight Hemostat, 5 1/2, 1 Posi-Grip Thumb Forceps. Latex free.

For Business Accounts:
- Pharmacy or wholesale distributor license issued by the state board
- License address must exactly match the shipping address
- License must be from the same state as the shipping address
For Individual Practitioners:
- Doctor, dentist, or nurse practitioner license issued by the state board
- Address does not need to match, but license must still be from the same state as the shipping destination
