Kit includes: 10" x 15" header bag, 4"x4" 8-ply gauze sponges, 2"x2" 8-ply gauze sponges, PVP 3s swabsticks, alcohol 3s swabsticks, 10cc luer-lok syringes, 20c luer-lok syringes, 16G x 1" needle, 25G x 5/8" safety needle, 18G x 1" safety needle, 15" x 15" steri-drape EENT drape, 9" x 5" x 2" tray, 24" x 24" CSR wrap.
Accepted Licenses
For Business Accounts:
- Pharmacy or wholesale distributor license issued by the state board
- License address must exactly match the shipping address
- License must be from the same state as the shipping address
For Individual Practitioners:
- Doctor, dentist, or nurse practitioner license issued by the state board
- Address does not need to match, but license must still be from the same state as the shipping destination
Note: A DEA license will not meet the requirement.
