"A quick-stick Latex-Free foam bandage that performs as well as latex bandages. Can be used for all cohesive bandage applications. Strong 20 lbs tensile strength. Quick-stick open cell foam clings to itself. Stays in place and will not unravel. Sticks over wet dressings. CellularFoam patented technology provides a long-lasting cohesive bond. Easy to apply. - License address must exactly match the shipping address

For Individual Practitioners:
- Doctor, dentist, or nurse practitioner license issued by the state board
- Address does not need to match, but license must still be from the same state as the shipping destination
