Patient chart tape are imprinted with Room No., Patient, Doctor and available in 17 colors; please specify when ordering: 1-White; 2-Yellow; 3-Green; 4-Pink; 5-Red; 6-Orange; 7-Blue; 8-Tan; 9-Salmon; 10-Copper; 11-Rose; 12-Violet; 13-Lavender; 14-Grey; 15-Lime; 16-Aqua; 17-Chartreuse. Supplied on a 500 roll with 1 core, removable adhesive for easy removal from charts.

Accepted Licenses
For Business Accounts:
- Pharmacy or wholesale distributor license issued by the state board
- License address must exactly match the shipping address
- License must be from the same state as the shipping address
For Individual Practitioners:
- Doctor, dentist, or nurse practitioner license issued by the state board
- Address does not need to match, but license must still be from the same state as the shipping destination
Note: A DEA license will not meet the requirement.
