2" Molded Binders Binders holds 350 + sheets and has a patented twin hinge design with a 5 year guarantee. Top open comes in 2, 3 or 5 rings, and Side open comes in both 3 or 5 rings and features 14 colors to choose from: Blue, Med Blue, Beige, Burgundy, Mauve, Plum, Buby, Aqua, Forest Green, Grey, Lilac, Lt. Blue, Seafoam Green, and Sienna.
Accepted Licenses
For Business Accounts:
- Pharmacy or wholesale distributor license issued by the state board
- License address must exactly match the shipping address
- License must be from the same state as the shipping address
For Individual Practitioners:
- Doctor, dentist, or nurse practitioner license issued by the state board
- Address does not need to match, but license must still be from the same state as the shipping destination
Note: A DEA license will not meet the requirement.
