Kit includes: 3╛" x 8" blank label case, 6" x 12" roll/750 pouch, purple textured, nitrile, yellow ear loop isolation mask, 2" x 2" split gauze IV sponge, 1" x 18" transpore tape, 6" cotton tip applicator, skin prep pad, 3mL ChloraPREP applicator, 4"x4" 8-ply gauze sponges, alcohol 3s swabsticks, 3╜" x 4╜" Tegaderm dressing, 20" x 20" wrap paper.

For Business Accounts:
- Pharmacy or wholesale distributor license issued by the state board
- License address must exactly match the shipping address
- License must be from the same state as the shipping address
For Individual Practitioners:
- Doctor, dentist, or nurse practitioner license issued by the state board
- Address does not need to match, but license must still be from the same state as the shipping destination
